Detail to Include in Sections of the OIR

Sources of information

Only list the sources of information used for the OIR.

Do not include those listed in the parent report (e.g. summary of facts, etc.).

Rehabilitative needs

Circle either Y or N for each need to indicate:

• if the probation officer has identified the need.
• if the offender agrees with the identification of the need.

Screening scores

Record the scores for:

• AUDIT
• DAST, and
• NINE.

Note; This information will be provided from the pre sentence report

Sentence management considerations

Record any of the following:

• observations about the offender and/or their associates (e.g. gang-related issues)
• observations about offender's attitude, motivations, etc.
• issues relating to management of offender
• information from other agencies
• offending not on the criminal history list (e.g. self-reports, other jurisdictions, agencies), and
• relevant information from family members, significant others, and employers.

Safety concerns

Include in this section:

• a summary of the offender's response to the screening questions in the short or full report
• any information about risk issues either to themselves or others
• any hearsay or unsubstantiated information, and
• information about aggression or assaults on staff or classes of people.

Final details

Record the:

• name of the report writer, and
• date the report was written.
