Complete the parole assessment report 

The following table provides an overview of the steps PS take to complete the parole assessment report.

Review the report and check for inconsistencies, errors and update with any new information, if appropriate.

The RR appendix should be attached to the parole assessment report, along with copies of the signed occupant and offender agreements.

If there are issues with the CPPS section of the report contact the service manager and ask to consider making amendments to the report.

Note: If the report needs changing the report will need to be ‘unfinalised’ by the IOMS helpdesk.

Interview the offender, explain the process and discuss the report.

Note: If the offender disputes factual evidence at this point, the sentence planner will contact the probation officer about the particulars disputed.

Encourage the offender to accept the report and ask them to sign it. If they refuse to sign the report, have this witnessed and signed as such by another corrections officer.

Sign the report and pass to the PCO sentence planning.

Review the report and pass to the manager sentence planning.

Review and approve the report and return to the PCO sentence planner.

On approval from the manager sentence planning, send the signed report to the local NZPB liaison officer who will then collate and send all the reports to the NZPB administrator.

Give a signed copy to the offender and place a signed copy on the offender’s prison file.
