Introduction

This topic outlines information about EM forms.

All requests for EM services from the EM company must be in writing via email using the approved EM forms.

Minimum notification periods

Forms must be sent via email to the monitoring company during working hours within the minimum notification periods, usually at least two full working days before the service is required.

Forms

The following table shows the purpose of each form and the length of notice the monitoring company requires.

Form

Description

CD1 New Start Notification

Use this form to request EM equipment and to advise the monitoring company of the details of a new CD sentence.

This form is also used to request new equipment when an offender moves out-of-area.

The form must be completed at least two working days prior to the first curfew.

CD2 Change of Details Notification

Use this form to advise the monitoring company of any permanent changes to the curfew address, curfew period or supervising CPPS staff member.

Note: This form is only to be used where an equipment swap-out is not required.

This form must be completed at least two working days prior to the change.

CD3 Faulty Equipment Swap-out Request

Use this form to request a swap-out of EM equipment when the original equipment is faulty.

This form must be completed as soon as practicable after a fault is identified.

CD4 EM Equipment Return Form

 Use this form to notify the monitoring company that equipment has been de-installed, recovered and is ready to send back to the monitoring company.

This form should be used in the following instances:

  • at the completion of CD
  • when an offender is taken into custody
  • when CD is appealed and the sentence is suspended
  • when an offender has moved out-of-area and new equipment will be installed at the new address, and
  • when CD has been cancelled.

The form must be completed once the equipment has been recovered and is ready to send back to the monitoring company. It must be completed as soon as practicable.
