Introduction

The offender plan document is created in IOMS. It:

  • contains information about the offender and the sentence(s)/order(s), and
  • sets out specific details of how the requirements of each sentence/order are to be met.

Note: There is a single offender plan for each offender. It includes information relating to each sentence/order that the offender is subject to. If the offender is subject to a rehabilitative sentence/order as well as other sentence(s) the probation officer supervising the rehabilitative sentence/order is responsible for completing the offender plan.

The following table outlines the information contained in the offender plan.

Records:

  • plan date, and
  • probation officer.

Records:

  • sentence/release type(s), and
  • key sentence dates.

special conditions

Records any special conditions imposed for the sentence/order(s).

Note: Standard conditions are not recorded in the offender plan, but it is the probation officer???s responsibility to ensure that the offender understands what these are. If any particular activities are required to implement/monitor compliance with standard conditions, these must be recorded in the offender plan.

key rehabilitative activities

Records key rehabilitative activities under:

  • programme
  • counselling, and
  • other.

key reintegrative activities

Records RI2 reintegrative activities under:

  • accommodation
  • employment
  • education, and
  • other.

key compliance activities

Records key compliance activities including:

  • reporting requirements
  • sentence management phase calculations, and
  • any activities required to meet special conditions that have not been recorded under rehabilitative or reintegrative activities.

Signatures of the:

  • probation officer
  • offender, and
  • service manager.

offender plan timetable

Records each planned activity with a start and finish date.

offender plan review

Records:

  • the offender plan review date, and
  • comments when the plan is reviewed.

offender plan objectives

Each objective is listed separately with:

  • comment linking assessment result with objective
  • comment on current situation
  • goals for offender
  • activities/strategies to address the need, and
  • objective reviews (date, progress, actions arising).

Note: It will not always be necessary to have multiple objectives in the plan. It may be that several activities which will address the identified needs and requirements of the sentence can be included under one objective.

Each activity is linked to an objective, and is listed separately with:

  • start and end date, and
  • who is to be responsible, e.g. probation officer, offender.
