Introduction

This chapter provides information to assist the probation officer when an offender transitions from one sentence to another. For example:

  • home detention (HD) to post detention conditions, or
  • parole to extended supervision.

The offender plan will not be terminated when the offender is transitioning from one sentence to another. Once entered into IOMS, details of the new sentence should populate through into the current offender plan. If this does not occur, the details should be entered manually.

Probation officer tasks

The probation officer will complete the following tasks when an offender is transitioning from one community-based sentence to another:

  • complete an event-based review of the offender plan
  • complete a sentence completion report (template available in Corrnet), for the first sentence (e.g. HD), and
  • un-assign the offender and re-assign to the new supervising probation officer, if required.

Liaise with relevant CPPS staff

If the new sentence/order is to be managed by another CPPS staff member, there must be liaison between the relevant staff members.

Additional objectives and activities

It may be necessary to add objectives and activities to the offender plan to ensure the plan addresses the requirements of the new sentence.
