Introduction

This topic covers the forms used for requesting and maintaining EM services provided by the monitoring company, Chubb NZ.

Important: All requests for services must be in writing using the approved forms listed below.

Minimum notification periods

Forms must be sent to the monitoring company during work hours within the minimum notification periods, usually at least one full working day before the service is required.

Forms

The table below shows the purpose of each form and the length of notice the monitoring company requires.

Note: All forms can be found on Corrnet under the Forms tab on the homepage.

Description

Electronic Monitoring Connection Request (CONN01)

Use this form to arrange for an offender’s connection to EM. The form includes a request for a dedicated landline if necessary.

Important: The form must be completed:

  • two working days before the proposed connection date, or
  • at least five working days before or as advised by the monitoring company at pre installation, if a dedicated landline is requested.

Note: If the offender is being considered for court, the connection notice should be sent to Chubb NZ, advising them to be on standby for a possible connection.

Electronic Monitoring Weekly Curfew Schedule (EMF01)

Use this form to set an offender’s weekly curfew schedule. It can be used:

  • for setting curfews that change from week to week, or
  • to set a permanent weekly schedule for an offender with regular approved absences.

Important: The form should be sent to the monitoring company during work hours at least one working day before the first curfew.

Note: The weekly schedule can be changed during the time it is in force by using EMF02 (see below). If the entire schedule is changed, send in a new weekly curfew schedule.

Electronic Monitoring Temporary Curfew

(EMF02)

Use this form for one-off changes to the weekly curfew schedule or one-off curfews.

  • If the form is used to change a curfew, it will only change the curfew for that day, and then revert back to the weekly schedule for the remaining days.
  • If you are cancelling a curfew, then enter NIL in the leave box for the day in question. This will cancel all the curfews for that day.

Important:

  • One working day’s notice should be given to the monitoring company when changing a curfew.
  • As little as one hour’s notice can be given for an emergency curfew.

Manual Monitoring Request (MANMON01)

Use this form to request manual monitoring of an offender outside the residence.

Manual monitoring should be discreet and the request should take into account that monitoring is done

  • via drive-by, and
  • within a range of 30 metres.

Important: This form should be sent to the monitoring company at least one working day before the request is undertaken.

Electronic Monitoring Service Request (SER01)

Use this form to request any extra services from the monitoring company such as:

  • pre-installation checks
  • range tests
  • change of bracelets, or
  • relocation of equipment.

Electronic Monitoring Disconnection Request (DISC01)

Use this form to arrange for an offender to be disconnected from EM.

Important: The form must be sent:

  • at least two working days before the disconnection date, or
  • as soon as possible if the offender is taken into custody.

Notifying of Change of Probation Officer (PO01)

Use this form to notify the monitoring company when an offender is transferred between probation officers.

This enables the monitoring company to keep up to date contact details for each offender.



Monitoring at specific locations

Where an offender is being electronically monitored at their residence, at work, or other specified location, separate curfew schedules must be sent for each unit even though they apply to the same offender.
