Introduction
A review of all offender plans must be carried out:
- at each time based review
- at the completion of the sentence/order
- when there is a significant event or change in the offender's circumstances (event based)
- when the offender transitions from one sentence/order to the next (eg, home detention to post detention conditions, parole with residential restrictions to parole after residential restrictions).
Purpose of the review
The purpose of the offender plan review is to:
- decide and record the offender’s progress against the objectives and activities outlined in the offender plan
- update the offender plan with any new information/details which relate to the management of the sentence/order
- ensure that the interventions being undertaken are achieving the aim of lowering the offender's risk level
- establish what should be achieved during the next review period.
Time-based reviews
Time-based reviews are completed in line with sentence requirements at set intervals from the start of the sentence/order.
The following table shows the frequency for time-based reviews of an offender plan for each sentence/order.

Note: Extended supervision offender plan reviews must be completed or reviewed in IOMS within one month of the extended supervision order starting, regardless of whether special conditions have been set.
Event-based reviews
Event-based reviews are completed when:
- there has been a significant change in an offender’s circumstances such as further offending
- the offender transfers to another Community Probation & Psychological Services (CPPS) service centre
- the offender is transitioning from one sentence to another (eg, home detention to post detention conditions)
- a judicial monitoring report or a New Zealand Parole Board (NZPB) progress report is completed (if it does not coincide with a quarterly review).
Responsibilities
The probation officer is responsible for:
- completing all reviews for offenders that they supervise
- ensuring that all information recorded in the review is correct.
The probation officer managing the rehabilitative sentence is responsible for the offender plan if the offender is subject to dual/multiple sentences.
This includes consulting with other CPPS staff (responsible for managing the other sentences) for input when undertaking a review of the plan.
The service manager is responsible for undertaking quality assurance checks on the reviews to ensure they are completed according to requirements.
