Steps to take

The following table outlines the steps to take to carry out a review of the offender plan.

Gather information for the review focusing on the offender’s progress against the offender plan. Sources of information should include:

  • an interview with the offender
  • information recorded in IOMS casenotes
  • feedback from the programme provider/counsellor (if appropriate)
  • consultation with other CPPS staff (if dual/multiple sentences)
  • consultation with other agencies (where appropriate), and
  • consultation with family/whānau/employer/support people (where appropriate).

Discuss the following with the offender:

  • progress (or lack of progress) to date against the objectives and activities in the offender plan, including:
    • objectives achieved/outstanding
    • activities achieved/outstanding, and
    • compliance with special conditions
  • progress with counselling/programme
  • any positive or negative events that have occurred during the review period
  • any proposed changes to be made:
    • in special conditions to meet the requirements of the sentence/order, or
    • in response to any change in circumstances (e.g. application required for additional or variation of conditions)
  • any further offending/charges that have occurred, and
  • the assessment of the offender’s current potential to re-offend.

Complete the offender plan review in IOMS, including:

  • update of the RoC/RoI
  • review comment noting overall offender compliance and progress with sentence/order (probation officer & offender)
  • progress comment for each objective/activity
  • action arising for each objective (if applicable)
  • objectives and activities revised/updated (where applicable), and
  • reporting/phase requirements for the period leading up to the next offender plan review.

Note: As all offender plan reviews are recorded in IOMS, no paper copy of the review is required to be retained in the sentence file.

Record in IOMS casenotes that the review has been completed.

Take any action(s) required resulting from the review.
