Follow these steps to complete an extended supervision order in addition to the procedure for all sentences/orders.

Schedule a formal review if one has not been carried out in the last two months. If the expiry date is known in advance, schedule the formal review two weeks prior to the expiry date. Reference: This Part, Chapter 7 Formal Review for Extended Supervision. |

As a part of the formal review, detail the offender's high risk situations, risk factors and mitigating strategies. |

3 |
Notify any external agencies involved in information sharing about the offender that the order has been completed. |

4 |
Notify the victim notification register (VNR) coordinator that the order has been completed. |
