Introduction

This topic outlines the information that the sentence completion report should provide and details the sections of the report.

Sentence completion report

The sentence completion report should provide:

  • a summary of the offender’s response to the sentence/order
  • comment regarding the achievement of the offender management plan objectives relating to the sentence/order being considered for completion
  • relapse prevention strategies
  • any remaining areas of concern
  • a summary of non-compliance issues
  • confirmation of other sentences the offender remains subject to, and
  • comment regarding the likely benefit of future community-based sentences.
Sections of the report

This table outlines the sections of the report and what information to include in each section.

Note: The sentence completion report is a Word document that should be cut and pasted into the offender’s IOMS casenotes upon completion.

Section

Information to include

Offender’s details

  • name
  • personal reference number (PRN), and
  • date of birth.

Sentence details

  • type of sentence/order
  • start date
  • comlpetion date
  • standard conditions, and
  • special conditions.

Response to sentence

  • any identified issues
  • interventions employed during the sentence and offender’s response, and
  • if the offender has not complied with the standard or special conditions, briefly explain.

Relapse prevention

  • remaining areas of concern relating to offender’s likelihood of further offending
  • arrangements for ongoing community support, and
  • reintegrative strategies discussed with offender to reduce further offending.

Further conviction

  • any further convictions while on sentence, including offence or sentence outcome, and
  • comments relevant to any action taken following re-offending (e.g. recall, breach, variation/cancellation).
Compliance
  • compliance with standard conditions
  • compliance with special conditions, and
  • in the case of non-compliance, details of any action taken (e.g. recall, breach, variation/cancellation)

Other sentences

any other sentences that offender is subject to, including the sentence type and completion (finish) date.

Suitability for subsequent sentences

  • whether or not the offender is suitable for subsequent community-based or home detention (HD) sentences, indicating “Yes” or “No”, and
  • any relevant comments.
