Introduction

This topic provides guidelines to assist the probation officer in completing the transfer assessment.

Timeframe

The transferring probation officer must complete a transfer assessment in time for the first report to the receiving service centre.

Make notes clear

As with all offender information, it is important to ensure that casenotes are clear. Casenotes should not contain personal shorthand, local abbreviations or reference to others involved with the management of the sentence without identifying their role in relation to the offender.

Offender???s response

The following information should be recorded in casenotes:

  • overall response to the sentence/release
  • progress toward achieving the offender plan goals, objectives, and any event-driven review
  • reasons for, and details of, any enforcement action taken
  • reporting patterns and reasons for increases or extensions
  • any areas of specific difficulty or recurring problems, and
  • notes on any:
    • evidence of changes in attitude/motivation
    • evidence of changes in lifestyle, and
    • ups and downs or crises.
Event-based offender plan review

An event-based review of the offender plan should be completed prior to the transfer and record ???transfer??? as the event. The review should contain:

  • the offender???s response to, and compliance with, the sentence requirements, and
  • progress against the sentence plan.
