This topic outlines the procedure for preparing a sentence completion report.
A sentence completion report must be prepared for all offenders subject to CW.
The following table outlines the steps to take to prepare the sentence completion report for CW.

When the offender is due to complete 75% of their CW hours, send them a written instruction to report (ITR) directing them to report to the service centre for interview. Note: In the case of centre-based placements, schedule the interview for the end of the offender’s usual CW day. |

Make appropriate arrangements with other staff supervising those sentences. |

Interview offender at the community work centre (CWC) in accordance with the ITR sent to the offender. Note: Ensure that the time taken for the sentence completion interview is recorded against CW hours in IOMS. |

Prepare the sentence completion report using the CW6 Sentence Completion Report Template. Copy and paste the completed report into IOMS casenotes. |

Note: Information about conducting a file review, interviewing the offender and writing the report is outlined below.
Before preparing the sentence completion report, a thorough review of the offender’s CW file (including IOMS records) should be undertaken.

Review the offender’s response to the sentence and their compliance with the conditions of the sentence. Note: If the offender has completed, or is also completing, a BWLS programme, consideration must be given to the offender’s performance and progress with this programme. |

Assess the offender for entitlement for up to 10% remission of hours. |

Determine the offender’s suitability for subsequent sentences of CW. |

Issue the offender with an ITR to obtain and sign their sentence completion report. The ITR should be issued for a date prior to or at the scheduled completion of their CW hours. Note: In the case of centre-based placements, schedule this for the end of the offender’s usual CW day. |

Finish the interview appropriately. |

Record the time taken for the sentence completion report interview against CW hours on the daily record sheet in IOMS. |
