Introduction

This topic covers the contents of the transfer assessment, which is prepared when:

  • an offender moves residence, and
  • the responsibility for management of the sentence is transferred to another CWC.
Deadline

A transfer assessment must be completed by the transferring senior community work supervisor/probation officer in time for the first report to the destination CWC.

If the offender reports to a new CWC without having discussed this with the senior community work supervisor/probation officer, then the transfer assessment should be completed within 5 working days of receiving advice from the receiving service centre of the transfer.

Make notes clear

As with all offender information, it is important to ensure that any case notes are clear, i.e. not in personal shorthand.

Offender’s response

In the notes, against the sentence in IOMS, include the following information about the offender’s response, as reported by, or discussed with, the offender:

  • compliance with reporting requirements
  • response to sentence overall
  • any areas of specific difficulty or recurring problems
  • details of and reasons for any current enforcement action, and
  • details of and reasons for any enforcement action already taken on current sentence.
