Introduction
This topic explains how to refer an offender to Work and Income or a programme provider for an assessment of their indicated BWLS needs.
Procedure
Follow these steps to make the referral for BWLS assessment.

Complete a referral for assessment letter to Work and Income or the programme provider, using one of these templates:

Complete the following sections of the Provider Assessment Confirmation (PAC) form (available on Corrnet):
- offender’s details
- CPPS staff member details, and
- provider/agency details (where these are known).

- Attach the partially completed PAC form to the referral letter and include a stamped addressed envelope, and
- send this to Work and Income or the programme provider.

- Copy and paste the referral letter into IOMS casenotes
- make a note in IOMS casenotes to follow up the PAC form if the completed form, or a copy of the letter confirming the assessment arrangements (date, time and place), has not been received within 2 weeks, and
- diary ahead the two week date for return of the letter and/or PAC form.

| On receipt of a copy of the letter to the offender: |

- record in IOMS casenotes that the letter has been received and record when the assessment is to take place
- put the letter onto the offender’s hard copy file
- if the offender is reporting before this date, make a note to remind the offender of the assessment, and
- note when the PAC form should be returned following the assessment and diary ahead the one week (post assessment) date for the return of the PAC form.
