5.1 All incidents
Notifiable incidents listed in 2.1, 3.1 and 4.1 above must be reported using the CPPS Initial Incident Report form attached to an email addressed to “CPPS Incidents”, except where they are completed by psychologists. In this case notifiable incidents must be reported to the Director of Psychologists.
5.2 Multiple categories to be reported
The incident category is determined by reference to the incident categories (2.1, 3.1 and 4.1 above). A report may have multiple incident categories; for example an offender may have assaulted a staff member resulting in the call for emergency services. The report must describe both the actual violence against the staff member as well as the call for emergency services to ensure that all incidents are captured for analysis.
5.3 Reporting timeframes
All incidents must be reported to head office on the IIR form within 24 hours of CPPS staff first becoming aware of the incident. Very serious incidents must be reported immediately to the Policy and Operations team at Head Office by phone. If a service manager is unsure whether an incident needs verbally reporting straight away, they should speak to their area manager or the Policy and Operations team for more advice and guidance. Incidents in categories such as O4, S1 and S5 will usually need to be reported immediately but each incident and the level of risk needs to be considered and assessed individually.
5.4 Action taken at Head Office
On receiving an Initial Incident Report, CPPS Head Office will:
The initial incident reports are forwarded to the Advisor Incidents email group (or to the HR-related Advisor Incidents email group if it is staff related). Serious incidents will also be forwarded to the Minister, who also receives a weekly summary of all IIRs each Wednesday.
