Observing an At Risk Prisoner
- Prisoners identified as at-risk who do not have a management plan are observed by way of physical sightings (not by camera) at intervals of not more than 15 minutes until a plan is developed.
- Once a management plan has been developed for an at-risk prisoner, they are observed by way of physical sightings (not by camera) according to the individual management plan for that prisoner. The observation interval specified in the plan may be shortened or lengthened if staff dealing with the inmate consider it warranted. To lengthen the level of time between observations requires the unit manager/on- call manager or delegated officers written authority.
- Observation sheets for the 24-hour period of the previous day are to be reviewed and signed by the unit manager or their delegate within each 24-hour period. Behavioural patterns are to be noted for referral to appropriate support personnel. Observation sheets are to be placed on the prisoner’s at-risk file.
Suicide prevention bedding and clothing
Staff managing areas of the prison site that have bedding and clothing designed to assist with suicide prevention, including Special Needs Units, Dedicated Care Units, At-risk Units etc., must:
- Check the bedding and clothing daily for signs of damage and deterioration in the fabric.
- Act on any suspicion of tampering by prisoners by taking closer inspection, and if need be, removing and replacing the damaged item.
