Required forms
The following sets out the required forms for prisoner releases:
Note: A Release Licence is issued upon release to all prisoners, except prisoners released from a short-term sentence, if on release, the offender is not subject to any release conditions.
- Release licence for offender subject to short-term sentence who is released from prison or home detention on court-imposed conditions. (C04.01.F1)
- Release licence for offender subject to long-term sentence released on parole from prison or from home detention. (C.04.01.F2)
- Release licence for offender released on compassionate release. (C.04.01.F3)
- Release licence for offender subject to long-term sentence released from prison or home detention at Statutory Release Date. (C.04.01.F4)
- A Detention Licence for offender on home detention is issued to every prisoner who is to continue serving their sentence on home detention. (C.04.01.F5)
- Advice to Prisoner Subject to of Cumulative Community Based Sentence. (C.04.01.F6) (Note: this only applies to prisoners sentence prior to 30 June 2002).
The forms are available on IOMS and all parts of the forms are completed correctly.
- Where prisoners are released from a sentence of imprisonment of more than 12 months, or from home detention or from an indeterminate sentence, or a sentence of 12 months or less with court imposed conditions the following forms are forwarded to the appropriate local office of the Community Probation Service:
- Appropriate Release Licence (C.04.01.F1 /F2 /F3 /F4 /F5)
- Receipt for Release on Licence (C.04.01.F8)
- Notification of Release on Licence (C.04.01.F9)
- Where prisoners are released from prison and are subject to a cumulative community based sentence imposed prior to 30/6/2002:
- Notification of Release on Licence (Sentence of imprisonment 12 months or less followed by a community based sentence). (C.04.01.F10)
- Release Licence (C.04.01.F1)
- Receipt for Release on Licence (C.04.01.F8)
- Advice to prisoner subject to cumulative community based sentence.
