1. Prisoner related incidents
All prisoner related incidents listed in section E.08.01 must be reported using the Integrated Offender Management System (IOMS) when available, or manually by using the E.08.01.F1 Notification of Incident involving Staff (National) form. In urgent situations, as indicated under section 4. Reporting time codes below, immediate notification by telephone is required as well.
2. Staff related incidents
All staff related incidents listed in section E.08.02 must be reported manually using the E.08.01.F1 Notification of Incident involving Staff (National) form. Reporting time codes refer to E.08.02. The Integrated Offender Management System (IOMS) is not to be used for staff related incidents.
3. Multiple categories to be reported
The incident category which is required to be provided must be determined by reference to the Incident Categories (section E.08.01 and Section E.08.02). A report may have multiple incident categories, for example a prisoner may have escaped and assaulted a staff member in the process. The report must list both the escape as well as the assault incident to ensure that all incidents are captured for analysis.
4. Reporting time codes
Incidents with an identified requirement for notification refer to the reporting time codes below:

5. Reporting time codes for categories
Refer to E.08.01 Incident Categories (with notifiable flag) for a list of the different incident categories and the related time codes for notification.
6. PECCS Reporting Requirements
The PECCS Manager is to advise the Department of Corrections PECCS Security Monitor of an incident, who will advise and provide an interim report to the Department of Corrections Operation Advisors as soon as practicable. The full incident report will be completed where practicable before the end of the Security Officer witness shift, or within 24 hours of the incident.
