Procedure Standard

  • There is early relapse intervention of prisoners who have previously been identified as posing a suicide risk.

Procedure

  1. All prisoners who have been released from HRAT after their seven-day observation period will be assessed by either the psychologist, psychiatric nurse or assessor in two weeks time.
  2. The prisoner is then assessed on a monthly basis by a member of the VRP team, until their release from ACRP.
  3. A diary is kept to maintain a track of prisoner's appointments.
  4. In the event that the prisoner presents with signs and symptoms of an increased risk to themselves then the HRAT process will intervene.
  5. All interviews are recorded in the prisoner’s medical chart.

Key Roles and Responsibilities

Psychiatric Nurse/Psychologist

  1. Review all prisoners recorded in the diary as to their current risk/self harm status.
  2. Record all interviews in the prisoner's medical file.
  3. If a prisoner is viewed as "at risk", implement the HRAT procedure.
