Procedure Standards

  • All prisoners are assessed to determine their Immediate Needs and At Risk Status;
  • All prisoners are provided with initial information of the prison’s routines and procedures;
  • Prisoners returning to ACRP from court will undergo the At Risk Assessment only;

Procedure

  1. Where Reception staff are advised or observe a prisoner displaying at risk behaviour the assessor and/or nurse are to be advised.
  2. Within four hours of reception into ACRP, all prisoners will be formally assessed for Immediate Needs and At Risk.
  3. Within four hours of reception into ACRP, all prisoners will have a Health Reception Screen completed.
  4. Prisoners will have the opportunity to access Maori and Pacific Island Liaison Officers to assist during assessment, where requested.
  5. The assessment will be completed in accordance with the Immediate Needs and at Risk format and entered into IOMS by the Assessor.
  6. Where a prisoner is identified At Risk he is to be referred to the Nurse and Assessor.
  7. Where two ‘yes’ answers are generated but the prisoner is deemed not at risk by the Assessor and Nurse, the Unit Manager will co-sign the At Risk form to that effect.
  8. Where a prisoner is assessed as not "At Risk", but has been "At Risk" during previous term of incarceration, this is to be recorded in the Vulnerability Assessment (hard copy local form) on the prisoner who is then referred to the psychologist.
  9. Prisoners identified as not At Risk are placed in an Accommodation Unit.

Key Roles and Responsibilities

Assessor

  1. Within four hours of reception, Prisoners are to be assessed in accordance with PPM including local procedures.
  2. Where an Prisoner is identified At Risk, immediate referral to Health Services. The assessor is to arrange a designated person to remain with prisoner until a Nurse has seen the Prisoner.
  3. Where an prisoner is assessed as not "At Risk", but has been "At Risk" during previous term of incarceration this is to be recorded in the Vulnerability Assessment on the prisoner who is then referred to the psychologist.
  4. Enter prisoner assessment details into IOMS (NARA and immediate checklist and designated screens) within four hours on the prisoner’s reception.

Nurse

  1. Where an prisoner is assessed as At Risk, implement HRAT process in accordance with local procedures.
  2. Complete Reception Health screen (IOMS) on prisoners within 4 hours of their reception.
  3. Complete both the initial and full Health Assessment (IOMS) within 24 Hours of the Prisoner’s reception.

Corrections Officer

  1. Where observed or advised of an prisoner displaying At Risk behaviour, immediately advise the assessor and/or nurse.
  2. On completion of reception and initial assessment and considered Not At Risk, escort prisoner to allocated accommodation unit.
  3. Where prisoner is identified as At Risk escort to Special Needs Unit.
