Procedure Standard

  • Current prisoners who may become At Risk are identified and their immediate needs are addressed to minimise the possibility of Self Harm.

Necessary Forms and Requirements

Forms:

  • B.14.03.F1 Local Reassessment Form (Local Form)

Requirements:

  • Nil

Procedure

  1. Staff receive information or observe behaviour that indicates a prisoner may become At Risk of Self Harm.
  2. Through discussion with the prisoner, staff attempt to get confirmation from prisoner of possibility of Self Harm.
  3. Form B.14.03.F1 is completed.
  4. If prisoner is assessed as becoming At Risk, 2 or more yes answers, or, it is considered that the prisoner is at risk, prisoner is referred to Health services.
  5. Health Services Staff take appropriate action to minimise Risk of Self Harm, document their actions and sign the form which is placed on the prisoner’s At Risk File.
  6. Where a prisoner is deemed as At Risk on the Local Re-assessment Form, the Nurse will carry out a Suicide Assessment and complete Part B.
  7. If it is assessed that the prisoner is not likely to become At Risk, Form is forwarded to prisoner’s Unit Manager who once satisfied with outcome, places form on prisoner’s file.

Key Roles & Responsibilities

Staff

  • Receive information or observe behaviour of prisoner becoming At Risk
  • Discuss with prisoner to establish confirmation
  • Complete B.14.03.F1 Local Reassessment Form
  • Refer prisoners considered At Risk to Health services

Health Services Staff

  • Take appropriate action to minimise Risk of self Harm
  • Document actions
  • Carry out Suicide Assessment where appropriate
  • Develop Interim Risk Plan
  • Complete Part B of Local Reassessment Form
  • Inform HRAT members

Unit Manager

  • Sign Interim risk Plan
  • Arrange placement for prisoner in Special Unit
